Already Enrolled - use the "Forgot Username?" link to reset your Username/Logon ID and Password. Use the "Forgot Password?" link to reset only your password.
Not Enrolled - to self enroll, use the Create Account tab. You must use the same email address you provided us during any visit(s) you have had at our facilities. It must match what we have on file. If we do not have an email address on file for you, you will not be able to self enroll. Please contact our Health Information Management department at (660) 890-7133 on Monday – Friday between 8 AM and 4:30 PM and they should be able to assist you.
